You also grant The Club permission to use your personal data as submitted by you for the purpose of conducting club business, as described by our privacy policy.

Please ensure to highlight any relevant medical issues or areas of concern that may impact your or family members’ enjoyment of Tennis.

With respect to parents/guardians, you agree to inform an authorised Club representative of any important changes to your child’s health, medication or needs. In the event of illness, having parental/guardian responsibility for a child, you give permission for medical treatment to administered where considered necessary by a nominated first aider, or by suitably qualified medical practitioners. If the parent/gaurdian cannot be contacted and should your child require emergency hospital treatment, you authorise a qualified medical practitioner to provide emergency treatment or medication.
